Jonathan Michael Vlasak, 35, a licensed agent authorized to sell accident insurance, issued fraudulent insurance policies and attempted to obtain approximately $8,200 from Occidental Life Insurance Co. and $3,750 from Americo Life Insurance.
“This was an emergency, and I commend our agents for continuing the investigation and uncovering the fraudulent scheme, - said State Insurance Commissioner Mike Causey. - Consumers must be especially vigilant to protect sensitive information.”
Department investigators charged Vlasak with 25 counts of forgery, 25 counts of identity theft, 22 counts of obtaining property by false pretenses and two counts of attempting to obtain property.
Vlasak has been taken into custody this week but released on $100,000 bail.
According to the National Association of Insurance Commissioners' license check website, Vlasak has been licensed since 2017.
